REVISED FORMS: (Posted 02/06/03 - 1 Item)

The Examination Answer Sheet, ANSC-7010 (2-03) - CG-4886, is now reformatted for printing on users systems. Notice that the pattern has been rotated 90 degrees so that printers that crop edges will not miss part of the pattern. The pattern itself matches the 11-76 edition- only the orientation is changed. Also, note the 8" line-- users must make sure their printer images this line at 8" long, or the pattern will not match existing answer keys. At the present time, there is no plan to offer this form as fill-in. The previous edition may be used.

(Posted 02/05/03 - 1 Item) UPDATE 2/6/03:

The Activity Report- Mission, ANSC-7030 (2-03) has been updated to include PE course completion information. Users of this edition will no longer need to submit ANSC-7033 to report PE statistics. Members reporting PE activity other than the last mission for a class must not enter data in the course completion section, with only one member making the entry for each completed class. The previous edition (1-03) may be used until supplies are exhausted; form 7033 would have to be used also to report class completion with that edition. When supplies of the 1-03 edition are exhausted, form 7033 will become obsolete. NOTE: You will notice that the requirement to enter whether the course was a "state" course has been eliminated; users selecting mission 04U are indicating by that selection that it was a "state" course.

The electronic file is updated to correct a problem with entering the date using the "TODAY" button. Also, will now properly check for correct two-letter abbreviation for the "State". Checking with ctrl-d will show the revision to be 002- this will also appear in the upper right corner of the form.
